20% of patients with bipolar disorder have OCD. It presents a clinical challenge as SSRIs can induce mania, cause rapid cycling, or mixed states. Possible treatment options outside SSRIs include topiramate and lamotrigine both supported by small RCTs. Aripiprazole and quetiapine can also be use. Odansetron can also be used and has the added benefit of reducing binge drinking.
